The NDTG has announced that the CCDO/CSCS Demolition Card Application form for New Applications/Upgrades/Renewals has been redesigned "In an effort to make CCDO card application process easier."
You can download the new form from the NDTG website.
While the form may have changed, the requirement that all applications must be accompanied with the appropriate touch screen test pass certificate and the Asbestos & Demolition Safety Awareness Certificate valid within the last two years has not.
